In South Africa, medicines, therapeutic peptides, compounded pharmaceutical products, and certain health-related substances are regulated by the South African Health Products Regulatory Authority (SAHPRA) in accordance with the Medicines and Related Substances Act, 1965.

The regulatory status of each product depends on the active ingredient, dosage form, route of administration, intended use, and whether a registered commercial equivalent exists in South Africa.

In addition, certain investigational or experimental peptide products offered through regulated compounding pathways may not be registered by SAHPRA for the indications described. These products may include peptides such as BPC-157, TB-500, AOD-9604, MOTS-c, SS-31 (Elamipretide), CJC-1295, Ipamorelin, Selank, Semax, DSIP, Cagrilintide, and other investigational peptide formulations or combinations. Such products are generally considered investigational, experimental, or off-label in nature and are supplied only following assessment by a registered medical practitioner and the issuance of a valid prescription.
Licensed pharmaceutical wholesalers may import active pharmaceutical ingredients (APIs) under the appropriate SAHPRA permits and regulatory approvals. Licensed compounding pharmacies may then prepare patient-specific formulations in accordance with applicable legislation, including Section 14(4) of the Medicines and Related Substances Act and Good Pharmacy Practice (GPP) standards.
Depending on the specific product, indication, and regulatory requirements applicable at the time of prescribing, additional regulatory authorisations, including SAHPRA Section 21 approval, may be required. The prescribing medical practitioner is responsible for determining the clinical appropriateness of a product, while pharmacies remain responsible for compliant sourcing, storage, compounding, dispensing, quality assurance, and maintenance of product integrity.
SAHPRA has repeatedly cautioned against the use of unregulated pharmaceutical and peptide products due to concerns relating to product quality, contamination, mislabelling, counterfeit products, and illegally imported substances. These warnings highlight the importance of obtaining medicines and therapeutic products only through legitimate, regulated channels.
